Employees can go to www.transamericaemployeebenefits.com and click on customers.

The Customer Log In screen will come up. If you have registered, you can input your user ID and Password and click on Log In. If not, you will need to click on Register. From there you should follow the instructions on the screen. Once you have registered, this will allow you to view policy information. If you have a life policy, you can see what your cash value is and so on.

Members pay less than $10, $20 or $40 for Preferred Brand and Generic Drugs.  Non Preferred Drugs are discounted an average of 19% off the average wholesale price.  Mail order is also available.
 
PPO Network Providers

Quick and easy search for doctors, hospitals and specialists in your area.  Remember, using a network provider is NEVER required.  You are free to use any provider you choose.  However, using a network provider will save you money.

Wellness Claim

If you are not assigning your wellness benefit to Quest Diagnostics for the comprehensive blood screening profile as your annual wellness test, submit your critical illness wellness claim by phone or fax!

File a wellness claim by Phone: 
Contact the Transamerica Worksite Marketing’s Claims Customer Service Department at (800) 251-7254  (Note: Wellness benefits for breast MRIs must be submitted via fax or mail)

File a wellness claim by Fax:
Fax directly to the Claims Department at (866) 586-6528

Document must include:
-Insured’s name and policy number.
-Covered person’s name, date of birth and relationship to insured, if other than the insured
provider’s billing statement, which includes the test/procedure and the date that it was performed.
