
Group Supplemental Out-of-Pocket Medical Expense Insurance

comprehensive medical program designed to cover serious illness or
injury that contains deductibles, co-insurance and co-payments for which
insured's are responsible. The GAP plan has been specifically created
to help cover these out-of-pocket expenses.

Optional Riders
Minimum Group Size: Required participation is 50% for groups of 99 or fewer eligible employees with a minimum of 5 or 10 lives (depending on state requirements); or 25% (with a minimum participation of 50 eligible employees) for groups of 100 or more eligible employees. Eligible employees, for participation purposes, include only those employees covered under one of the employer’s underlying medical plans. .
